Information you provide
When you request a rate estimate, we collect your name, email address, optional phone number, and the calculator selections used to prepare the estimate, such as membership type, household tier, age range, IUA, and selected add-ons. When you request the free guide, we collect the name and email address you enter. Scheduling is completed through the external booking form, which collects the details you provide there. Do not enter private medical, financial, legal, or tax information into the site chat.
Information collected automatically
We may receive technical and attribution information such as the page you visited, referrer, URL parameters (including UTM campaign parameters), browser context, and limited display-preference data stored in your browser. The site uses analytics tools, including Google Analytics and Umami, to understand site performance and visitor activity.
How we use information
We use information to prepare and deliver a requested rate-estimate PDF and email, respond to a requested contact or booking, route and measure website leads, operate and secure the site and its source-guided chat, and improve website performance and educational content.
Service providers and external links
Rate-estimate requests are routed through GoHighLevel/LeadConnector and estimate emails and PDFs are delivered through Resend. Chat questions are processed to provide a source-guided answer. Analytics providers process site-usage information. If you follow an enrollment link to Zion HealthShare, you leave this site and are subject to Zion HealthShare's own notices and terms. We do not state retention periods, cookie practices, or privacy practices for third-party providers beyond the way this website sends information to them.
